Ability to multitask and prioritize. Must have great organization skills, written and oral skills and ability to present in front of small groups with the purpose of recruiting and have them seek their Health Care needs at Eisner Pediatric & Family Medical Center.
The role of the Outreach Community Specialist is to provide care coordination to support families entering the health care system and assisting with utilization and retention of benefits. This position focuses on contact with community members through the development of relationships and one-on-one interaction. The Outreach Community Specialist will coordinate in reach, outreach and enrollment efforts by working in collaboration with EPFMC staff, schools, parent organizations, community organizations, churches, and other entities. The Outreach Community Worker will have full knowledge and will keep abreast of all services, classes, and programs in the clinic therefore informing patients and assisting with all inquiries.
Qualifications and Education Requirements
High School Diploma or Equivalent
Customer service skills, Computer literacy, Bilingual English/Spanish, Medical Assistance Certify
